How to make an appointment
Having decided that you feel I may be able to help you with your problem, please
phone my secretary, Adrienne, on 03 9416 2470 (2
lines) and make an appointment.
Your appointment
Initial appointments are usually half an hour. Longer appointments
are allocated if you are seeking a second opinion, or if you
and your partner are attending an infertility consultation.
Follow-up appointments are usually a quarter of an hour. Again
longer appointments will be made for an infertility consultation
with your partner attending.
A referral from your General Practitioner
To arrange a consultation you will need a referral from your
general practitioner. If your partner will also be attending,
make sure they are also named in the referral letter, so that
you may obtain a specialist rebate from Medicare for both
of you.
Partner Participation
This practice encourages the participation of partners/husbands
in such issues as infertility management. To facilitate this,
please make sure that your partner/husband is named in the
referral from your doctor. A joint appointment will cost you
less (in out of pocket expenses) than an individual appointment.
However both you and your partner/husband will be billed for
the appointment, and both will need to claim for the Medicare
rebate.
Consultation fees
This practice does not bulk bill. Consultation fees are based
on those considered fair and reasonable by the Australian
Medical Association (AMA). As a consequence there will be
an 'out of pocket' cost.
If you have any further questions about fees, do not hesitate to ask.
Filling in Forms
To facilitate your consultation and make it as stress free and productive as possible, we ask that you complete some personal data, and medical information forms prior to your consultation.
Our standard forms can be downloaded from this website. You
can return the completed forms by email, by fax, or by mail.
When you make your appointment, Adrienne will advise you on
which forms are required, and will guide you through the website
to find them if you need assistance.
